Mr. Finch moved to accept the proposed work. Mr. Maughan seconded. Voted 5-0 in favor. 101 Willow Street, Austin Prep School, Order of Conditions, DEP 270-331, RGB 2000-4 The Commission reviewed the site visit report and the request for Certificate of Compliance and bond release. Mr. Finch moved to issue the Certificate of Compliance with long-term maintenance conditions attached. Ms. Longley seconded. Voted 5-0 in favor. Mr. Maughan moved to release the bond in full. Ms. Longley seconded. Voted 5-0 in favor. Public Hearing, Notice of Intent, John Pember, 7 Heritage Drive, RGB 2004-31 The Chairman opened the hearing. Mr. and Mrs. Pember were present. The Commission reviewed plans for a screened porch addition, modification of another porch, and grading. They discussed trees to be removed, roof rounoff, buffer zone restoration and setback, and soil stabilization. Mr. Maughan moved to grant a waiver of the 35-foot setback for structures due to the pre- existing developed condition and the lack of space on the site. Mr. Greene seconded. Voted 5-0 in favor. Ms. Longley moved to continue the hearing to October 27, 2004 at 7:30 PM. Mr. Finch seconded. Voted 5-0 in favor. Public Meeting, Request for Determination of Applicability, George and Maureen Schlehuber, 51 Mark Avenue, RGB 2004-32, The Chairman opened the meeting. Forest Lenthall, surveyor, Mike Marshall, applicant, and John Downey, contractor, were present. The Commission reviewed plans for_removal of a deck, and_construction of an additionand a new deck. Part of the driveway will be abandoned and filled. The Commission discussed trees to be removed, impervious cover, grading, , limits of work, soil stabilization and Minutes, Reading Conservation Commission, October 13, 2004 abandonment of the driveway catch basin. The Commission reviewed draft conditions for the work. Mr. Maughan moved to issue a negative Determination with the conditions as drafted to permit the project. Mr. Greene seconded. Voted 5-0 in favor. Public Hearing, Notice of Intent, Robert Tremblay, 15 B Street, RGB 2004-30, The Chairman opened the hearing. John Tremblay, the son of the applicant, was present. The Commission reviewed the proposal to remove a deck and construct a sunroom. The Commission discussed footings, runoff, impervious cover, and present site conditions. The Commission determined that some of the landscaping, part of the paved driveway, and other items were located on the abutting land where the wetlands were located. The Commission asked for additional information from the owner of the abutting land, Town Counsel, and historic land use records. Mr. Maughan moved to continue the hearing to October 27, 2004 at 7:45 PM. Mr. Greene seconded. Voted 5-0 in favor. Public Hearing, Request to Amend Order of Conditions, Nick Diranian, 15 Timothy Place, DEP 270-398, RGB 2003-16, At the request of the applicant, the Commission continued the hearing to October 27, 2004 at 8:00 PM. Public Meeting, Request for Determination of Applicability, David Godwin, 30 Bond Street, RGB 2004-33, The Chairman opened the meeting. Mr. Godwin was present. The Commission reviewed the proposal for construction of a deck, driveway, patio, retaining walls, and play yard. The Commission discussed the wetland boundary, local drainage system, and lot boundaries. The Commission reviewed and amended draft conditions. Mr. Greene moved to issue a negative Determination with the conditions as amended to permit the project. Ms. Longley seconded. Voted 5-0 in favor Public Meeting, Request for Determination of Applicability, Eshani Shah, 15 Beaver Road, RGB 2004-34, The Chairman opened the meeting. Mrs. Shah was present. The Commission reviewed the proposal to convert a deck to a sun porch. The Commission discussed the wetland boundary and soil stabilization. The Commission reviewed draft conditions. Mr. Maughan moved to issue a negative Determination with the conditions as drafted to permit the project. Mr. Greene seconded. Voted 5-0 in favor. OldlNew-Business- Walkers Brook Crossing, Order of Conditions, DEP 270-352, RGB 2001-14 Ed Shaw was present and requested occupancy for Jordan's on October 28. They are working on the final paving, fencing, drainage system, and landscaping for the parking lot. Minutes, Reading Conservation Commission, October 13, 2004 They have installed the liner under the detention basin in the phase 2 area and expect to install gravel around it before winter. They will be planting the trees and shrubs around the base of the retaining wall this fall. They would like to mow the grass planted between the edge of the Jordan's parking lot and the top of the retaining wall as a lawn. Mr. Finch moved to authorize the Administrator to sign off on the occupancy permit pending completion of the parking lot and detention basin. Mr. Greene seconded. Voted 5- 0 in favor. 115 Charles Street, Order of Conditions, DEP 270-367, RGB 2002-3 Mr. and Mrs. McCormack were present. The Commission reviewed an as-built site plan and an ANR plan for the property. They discussed the intrusion of the lawn into the Zone of Natural Vegetation and the extent of impervious cover. They requested that the McCormacks install permanent bounds to mark the ZNV boundary and plant 10-15 native shrubs to restore the area. They accepted the engineer's analysis of the infiltration capacity of the block pavers. Reading Memorial High School, Oakland Road, Order of Conditions, DEP 270-416, RGB 2003-51 The Commission discussed the status of work and the need for better inspection and supervision. Wood End Elementary School, Sunset Rock Lane, Order of Conditions, DEP 270-341, RGB 2000-38 The Commission discussed the status of work and asked for a site inspection with the parties responsible for completing the project. Johnson Woods, Order of Conditions, DEP 270-424, RGB 2004-10 The Commission reviewed pre-construction submittals and warrant articles for limited access over Longwood Road. 19 Dean Road, Determination of Applicability, RGB 2003-17 The Commission reviewed a site visit report. Work is complete. Mr. Greene moved to authorize sign-off on the occupancy and close the file. Ms. Longley seconded. Voted 5-0 in favor. 7 Applegate Circle Mr. Greene moved to accept the Minor Project Permit for reconstruction of the swimming pool patio. Ms. Longley seconded. Voted 5-0 in favor. 337 Ash Street The Commission decided upon a list of actions to be taken to correct the recent violation and directed the Administrator to issue them as an Enforcement Order. 422 Grove Street Mr. Greene moved to accept the Minor Project Permit for removal of a dead tree. Ms. Longley seconded. Voted 5-0 in favor. Minutes, Reading Conservation Commission, October 13, 2004 23 Fairchild Drive The Commission reviewed a site visit report. Work is complete. Mr. Finch moved close the file. Ms. Longley seconded. Voted 5-0 in favor. 46 A Street The Commission reviewed a site visit report. A notice of violation will be sent to the owner. Sumner Cheney Place, 32 Lucy Drive, 28 Smith Avenue 12 B Street, 41 A Street, 14 B Street The Commission reviewed site visit reports. Work is complete under Minor Projects or Determinations at each site. Mr. Greene moved to close the Minor Project and Determination files. Mr. Finch seconded. Voted 5-0 in favor. West Street Water Main Work is scheduled to begin soon for the water main to Johnson Woods. Birch Meadow Water Main Work has begun to replace the 20-inch water main under Birch Meadow. The contractor has not set proper erosion controls. The Administrator will contact the DPW. 275 Salem The Commission received a site visit report that the parking lot is being repaired as requested. 80 New Crossing Road, Order of Conditions, DEP 270-285, RGB 1996-9 The Commission received a site visit report that the flood storage compensation areas have been graded, seeded, and mulched. The owner will be contacted about other cleanup activities. 52 Munroe Avenue The Commission received a site visit report that the owner has continued with work to stabilize disturbed areas as requested. 25 Lucy Drive, Violation Notice The Commission received a site visit report that the shrubs had been planted in the ZNV. 34 Mill Street, Enforcement Order The Commission received a site visit report that work has been completed under the Enforcement Order. _ - -Mr. Finch moved to close the file. Mr. Maughan-seconded. --Voted 5-0 in favor. 4 Minutes, Reading Conservation Commission, October 13, 2004 553 Haverhill Street, Order of Conditions DEP 270-409, RGB 2003-38 The Commission reviewed a revised grading plan. Mr. Finch moved to accept the plan as an insignificant change under the Order of Conditions. Mr. Maughan seconded. Voted 5-0 in favor. The meeting adjourned.
