Minutes of the Reading Conservation Commissio , Selectman's Meeting Room, Town Hallt 12' ' 9 December 1, 2004 Present: Bill Hecht, Chairman, Will Finch, Doug Greene, Becky Longley, and Jamie Maughan, Commissioners; Fran Fink, Administrator The meeting was called to order at 7:00 p.m. Old/New Business: 1364 Main and Mass. Highway Department Bob Boone of NHD presented a plan dated 11/29/04 for repairs of the drainage system in Route 28 and a stream channel on private land at 1364 Main Street. Mr. Maughan moved to accept the plan as routine maintenance of the drainage system, provided that appropriate erosion controls are employed. Ms. Longley seconded. Voted 5-0 in favor. 40 Harvest Road, Order of Conditions, DEP 270-417, RGB 2003-53 Mr. and Mrs. Kenney, owners, presented an as-built plan and requested a Certificate of Compliance. The Commission reviewed a site inspection report. Mr. Greene moved to issue a Certificate of Compliance. Mr. Finch seconded. Voted 5-0 in favor. Mr. Finch moved to release the bond in the amount of $7,500.00. Ms. Longley seconded. Voted 5-0 in favor. Pondview Lane, Lot 5, Order of Conditions, DEP 270-345, RGB 2000-52 The Commission reviewed a site visit report. Letters will be sent to the developer and the owner of Lot 4. Mr. Maughan moved to authorize the Administrator to sign off on the Certificate of Occupancy. Ms. Longley seconded. Voted 5-0 in favor. Public Hearing, Notice of Intent, Cornerstone Ventures, LLC, 105 Libby Avenue, RGB 2004-36 The Chairman opened the hearing. The applicant, Paul Pandolfo, and the engineer, Bob Lynch, were present. Mr. Lynch presented a plan to demolish an existing house and construct a new house and driveway in Buffer Zone. The on-site septic system will be abandoned and a connect made to public sewer. The Commission asked questions about grading, trees to be saved, limits of work, and the process to abandon the septic system. Ms. Fink distributed written comments. An abutter stated that he had seen blue-spotted salamanders in the area. He did not have photographs. Comments have not yet been received from NHESP. 1 Minutes, Reading Conservation Commission, December 1, 2004 Mr. Greene moved to continue the hearing to December 15, 2004 at 7:30 PM. Mr. Finch seconded. Voted 5-0 in favor. Public Hearing, Request to Amend Order of Conditions, Nick Diranian, 15 Timothy Place, DEP 270-398, RGB 2003-16 The Chairman opened the hearing. The applicant, Nick Diranian, and the attorney, Josh Latham, were present. Mr. Latham reported that the Board of Selectmen had denied permission to leave the wall, fence, patio, and propane line in the public right-of-way. The Board ordered their removal within 60 days. They are exploring other options to permit these structures. Mr. Maughan asked for information about the gravel below the lower retaining wall. Mr. Diranian said he had a letter from the contractor but forgot to bring it. The gravel is to suppress plant growth. He would like the Commission to issue the Amended Order of Conditions based on the present site plan, with a possible revision in the future if the structures ultimately must be removed. The Amended Order had not yet been drafted, and the Commission considered various options. Mr. Maughan moved to continue the hearing on February 23, 2005 at 7:30 PM. Ms. Longley seconded. Voted 5-0 in favor. Old/New Business: 80 New Crossing Road, Order of Conditions, DEP 270-285, RGB 1996-9 The Commission reviewed the status of work and as-built plan. A letter will be sent concerning recent work at the loading dock ramp. Mr. Finch moved to issue a Certificate of Compliance with Conditions in Perpetuity. Ms. Longley seconded. Voted 5-0 in favor. 105 Arcadia Avenue, Determination of Applicability, RGB 2002-10 The Commission reviewed a proposed planting plan. A letter will be sent requesting native plant species. 20 Melendy Drive, Order of Conditions, DEP 270-391, RGB 2003-8 The Commission signed a corrected version of the Certificate of Compliance. 1 General Avenue, Order of Conditions, DEP 270-428, RGB 2004-22 Ms. Fink reported that the hazardous waste removal work is progressing and she will inspect the water main installation with the owner soon. 9 Mill Street, North Reading, DEP 245-1037 Ms. Fink and Mr. Finch will attend the pre-hearing conference in Boston. 335 Main Street The Commission reviewed a site visit report. The owner is working on plans for drainage and planting and expects to file a NOI this spring. 2 Minutes, Reading Conservation Commission, December 1, 2004 Wood End Elementary School, Sunset Rock Lane, Order of Conditions, DEP 270-341, RGB 2000-38 Work remains incomplete on the site. The School Department is working on revised plans for parking. Reading Memorial High School, Oakland Road, Order of Conditions, DEP 270-416, RGB 2003-51 The Commission discussed site visit reports and lapses in erosion controls. A follow-up site inspection will be made with Mr. Hecht and the contractor. A meeting is also being scheduled by the Town Manager with the Superintendent. Barrows School, Edgemont Avenue,, Order of Conditions, DEP 270-414, RGB 2003-49 Ms. Fink reported that the infiltration bed is being installed. 67 Forest Street Mr. Maughan moved to approve a Minor Project Permit. Ms. Longley seconded. Voted 5-0 in favor. Strout Avenue - Water Treatment Plant Mr. Maughan moved to approve a Minor Project Permit. Mr. Finch seconded. Voted 5-0 in favor. 293 Haverhill Street Mr. Greene moved to approve a Minor Project Permit. Mr. Finch seconded. Voted 5-0 in favor. 127 Charles Street After reviewing a site inspection report, Mr. Finch moved to close the file for the Minor Project Permit. Ms. Longley seconded. Voted 5-0 in favor. 7 Applegate Lane After reviewing a site inspection report, the Commission will send a letter to cleanup behind a shed. Maplewood Village Ms. Fink gave a site visit report. The construction sequence is expected for the December 15th meeting. 311 Haverhill Street The Commission reviewed a site visit, report and will follow up with concerns about erosion controls and grading. 2F Carnation Circle After reviewing a site inspection report, Mr. Finch moved to close the file for the Minor Project Permit. Ms. Longley seconded. Voted 5-0 in favor. Ms. Longley seconded. Voted 5-0 in favor. 30 Chequessett Road The Commission reviewed a site visit report and will follow up with concerns about brushpiles in the wetland. Hunt Memorial Park The Commission reviewed a revised site plan. Mr. Greene moved to accept the plan as an insignificant change under the Determination of Applicability. Ms. Longley seconded. Voted 5-0 in favor. Master Plan Mr. Finch reported that the Master Plan Advisory Committee has begun drafting the plan. Ms. Fink will provide some updated information for the open space and natural resources sections. 87 Walkers Brook Drive A letter will be sent to the Board of Selectmen concerning the status of the hazardous material cleanup at the site. Meeting Dates The Commission set meeting dates for 2005. The meeting adjourned. 3,r
