Town Hall, Conference Room
August 31, 2005 - 6:00 - 7:60 p.m.
1. Present: James Bonazoli, Lyn White, Anne Marie Bourque, Bob LeLacheur, Carol Patterson, Dawn
Folopoulos (Staff).
2. Minutes of the August 17, 2005 meeting accepted. Motion by James, seconded by Lyn.
3. Lynn provided data gathered from the 2005 Federal Census about Reading's elder population.
4. Anne Marie provided information about Parish Nurses. The focus appears to be the integration of
faith and health.
5. After much discussion about what a "Nurse Advocate" is, the task force developed this description of
the roles and responsibilities of a Nurse Advocate if we were to have one in Reading.
• Health Educator-focusing on a variety of educational activities and resources for elders and
caregivers- system and
• Personal Health Counselor - assess/evaluate a resident's health, safety, connectedness. Assisting individuals to deal with health concerns, which may include doctor,
hospital, home, nursing home, etc. visits.
M Health Advocate - providing information and referrals for health and wellness through
community resources.

Those towns included AndovernWinchestera„d Arlington some variation of a nurse advocacy
program.
7. Next Steps: populations in each of the towns
• Lynn will gather demographic information about the elder pop
listed above.
• Anne Marie and Dawn will share the task of contacting each of the Town's Elder Service
Directors to inquire about the Nurse Advocate including job descriptions, salary, and number
of elders served.
8. Discussion about how we define "frail.„ Should we as a task force be defining "frail?" Should we use
MVES's criteria for service eligibility as a definition? Should we remove the term "frail" from our task
force's mission. Do we need criteria to limit the use of nurse advocacy services if we had such
services in Reading?
9. Motion by Carol to meet at 6:00 p.m. instead of 5:30 p.m. for the remainder of our meetings and
seconded by Lyn.
10. Motion to adjourn by Anne Marie and seconded by Lyn
