Meeting of the Board of Survey opened at 7 :45 P. M. in Room
16, Municipal Building.
Present were Chairman Russell , Secretary Dustin, Board Mem-
bers Blood, Price and Botka and Superintendent Louanis.
Minutes of the previous meeting were accepted as corrected.
The Board reviewed the Preliminary plan entitled, "Riverside
Homes Preliminary Subdivision in Reading, Mass." dated October 12,
1976. It was moved, seconded and voted to disapprove the plan as
it involved town owned land and drainage and sewerage easement that
would require release and relocation by the Town of Reading.
Following a brief discussion it was moved, seconded and voted
to approve the definitive plan and profile of Countryside Lane
Extension in Reading, Ma. , owned by Chimo Development Corp. and
dated October 7, 1976 subject to the following conditions and
modifications:
' 1 . The minimum cellar floor elevations will be set for the pro-
posed lots and approved by the Engineering Division.
2. The existing ditch location in Lot 4c is very objectionable
in appearance. Improve the existing headwall and reduce the
existing ditch side slope 1 on 3 so it can be mowed and there-
fore easily maintained by the homeowner .
3. The increased drainage due to the modification is insigni-
ficant and if the suggested improvement mentioned in Item
2 is done the drainage system will be more than adequate.
4. The Town reserves the right to modify the ejector station
plans and specification.
5. All the appropriate drawings and information as required for
the Water Pollution Control and M. D. C. approvals will be
furnished by the developer.
6. Show gutter profile around cul de sac.
7. Make the 30 foot wide easement in lot 4c for future water
as well as sewer main extension.
8. The extension of the water main will be done by the Public
Works Department of the Town of Reading.
9. The street cross section shown on the plan will include the
required sidewalks, and:
Subject to the receipt of a Conveyance of Easements and Utilities
and Covenant Agreements if the developer ' s engineer satisfactorily
modifies the above items from the list to the satisfaction of the
Superintendent of Public Works they may be deleted from the
Covenant.

It was moved, seconded and voted to approve the definitive
plan of Milepost Road, Reading, Massachusetts owned by Crescent
Realty Trust, Washington Avenue, Burlington, Massachusetts and
dated August 31 , 1976 subject to the following modifications and
conditions:
1 . The minimum cellar floor elevations will be set for the pro-
posed lots and approved by the Engineering Division.
2. The lots being resubdivided to provide area for the new lots
will require some special attention and a new plan. Ref.
existing lots 20-24 excluding lot 22. Ref. existing Covenant
Agreement.
3. The exit headwall in lots 21A and 53 will be of the flared
exit type suitable installation.
4. The proposed profile plan indicates a fill section near sta-
tion 6+14 thus creating a pocket of water . This area may
therefore need a headwall to eliminate the potential problem.
If it does prove to be necessary the additional drainage and
drainage easement will have to be provided.
5• The existing 36" elm tree located on Hopkins Street is a poten-
tial site distance hazard but should remain until the actual
roadway is cleared so that an on site evaluation of its poten-
tial hazard can be made.
6. Every effort will be made to make the existing ditch clean and
excavated and it should blend into the surrounding area with ,
side 1 :3 slopes that will be loamed and seeded at a mowagle
cross section.
7. The extension of the water main will be done by the Public
Works Department of the Town of Reading.
The proposed plan locations and material type and number may be
revised by the Water Department Supervisor to conform to the Town ' s
installation specifications and construction procedures and subject
to the receipt of a Conveyance of Easements and Utilities and
Covenant Agreement. If the developer 's engineer satisfactorily
modifies the above items from the list to the satisfaction of the
Superintendent of Public Works they may be deleted from the Covenant.
The Board reviewed the explanation of legal documents for
Hopkins Farm appended hereto and it was moved, seconded and voted
to follow the recommendations of Mr. Bergeron.
The Board signed the following:
Release of all lots in Hopkins Farm from operation of
Covenant Agreement - Crescent Park Realty Trust, Salem Five
Cent Savings Bank and the Reading Board of Survey: Document #1615•
release of Restrictions - Crescent Park Realty Trust: '
Document #1616.
Release of Restrictions - Old Farm Road, Gateway Circle,
Haystack Road: Document #1617.
Release of Restrictions - Old Farm Road, Haystack Road,
Walnut Street and South Street: Document #1628.

Explanation of Legal Documents
for
Hopkins Farm
1 . All lots are covered under an agreement dated June 15,
1976 between J. L. Peabody at al and the Town of Reading.
2. The following lots are covered under a three way agree- ,
ment dated August 24, 1976 between Crescent Park Realty
Trust, Salem Five Cent Savings Bank and the Tot-in of Read-
ing for a sum of $55,600;
Old Farm Road: 1-11 , 33, 35-41 ;
Haystack Road: 32;
Walnut Street: 34;
South Street: 51 and 52.
3• The following lots were released from the sale and occupancy
restrictions of the agreement referred to in No. 1 ;
Old Farm Road: 1-11 , 33, 35-41 ;
Haystack Road: 32;
Walnut Street: 34;
South Street: 51 and 52•
4. The following lots were released from the sale and occupancy
restrictions of the agreement referred to in No. 2;
Old Farm Road: 1-11 , 33, 35-41 ;
Haystack Road: 32;
Walnut Street: 34;
South Street; 51 and 52.
5. 1 recommend the acceptance of an agreement dated December
13, 1976 for a sum of $55,600 covering the following lots;
Old Farm Road: 1-13, 33, 35-43;
Haystack Road: 23-26, 30-32, 44-49;
Walnut Street: 34;
South Street: 51 and 52;
Gateway Circle: 27-29.
6. 1 recommend the total release of the agreement referred to
in No. 2. (All lots are now covered under the new agree-
ment referred to in No. 5• )
7.z 1 recommend the release of the following lots from the
sale and occupancy restriction of the agreement referred
to in No. 5;
Old Farm Road: 1-13, 33, 35-43;
Haystack Road: 23-26, 30-32, 44-49;
Walnut Street: 34;
South Street: 51 and 52;
Gateway Circle: 27-29.
8. 1 recommend the release of the following lots from the
sale and occupancy restriction of the agreement referred
to in No. 1 ;
Old Farm Road: 12-13, 41-43;
Haystack Road: 23-26, 30-31 , 44-49;
Gateway Circle: 27-29
9. The net result of all the preceding (Nos. 5-8) will be
the addition of the following lots being placed under
the existing monies ($55,600) and the releasing of the
sale and occupancy restrictions of these same lots.

Haystack Road: 23-26, 30-31 , 44-49;
Gateway Circle: 27-29•
