Ms. Longley called the meeting to order at 7:00pm.
DOC 270-0664: 76 Longwood Road, Map 19 Lot 15, Mahony
The OOC was reviewed by the Commission and the consultant with no additional comments.
Motion to Issue DOC 270-0664 by Mr. Flynn, second by Mr. Pinette. Vote 5-0-0.
COC 270-0633: 118 Willow Street, Map 20 Lot 224, Sellitto
Mr. Tirone reviewed his site visit. The plan called for an addition, removal of one of two
shrubs, and a driveway. There was no erosion and the grass came in fine.
Motion to approve COC 270-0633 by Ms. Scanlon, second by Mr. Flynn. Vote 5-0-0.
Enforcement Order: 218 Salem Street, Map 23 Lot 90, Yonjan
Ms. Longley reviewed the site visit with Tom Hughes, the homeowner's biological wetlands
consultant. Some wetland Flags were adjusted in the field. The applicant would like to
make changes to an existing retaining wall that borders the 25-foot line Including: removing
part of it and bring in loam to regrade the area; and changing a part of it that will encroach
into the 25-foot zone, which Mr. Tirone was adamant that they not do. Mr. Hughes will be
submitting an updated plan

Mr. Pinette. Vote 5-0-0.

ANRAD 270-: 128 Eaton Street & Lakeview Avenue, Map 7, 18 Lot 274, 275, 276,
131, 1 & 2, Fodera
Bill Manuel of Wetlands & Land Management was present to review the wetland boundary
and property at the intersection of Eaton Street and Lakeview Avenue. He reviewed the site
visit with the Commission. The bordering land subject to Flooding is at the 84-foot elevation
line. He positioned the flags along the top of the bank and along the high-water line instead
of having two separate lines only a foot apart. Because Walkers Brook is historically
channelized and straightened, the bank is very steep in some locations.
Mr. Tirone noted that he found some endangered burweed in the area and contacted the
National Heritage. He will submit photos of the plant, which will be identified by a state
botanist, and wait for a response.
Lawrence Rocheleau of 108 Eaton Street asked what the plan is for the property. Mr.
Manuel stated that the proposal is for apartment buildings.
The Commission agreed that more information would be needed including: another site visit
to take photos of plants and look at wetland lines subject to flooding; plan updates to clarify
the top of the bank and add the 100-foot Inner riparian; and missing pages from the cover
letter.
Motion to continue ANRAD 270-: 128 Eaton Street & Lakeview Avenue by Ms.
Scanlon, second by Mr. Curtis. Vote 5-0-0.
270-0568 RGB 209-36: Extension, Town-Wide Public Trails Maintenance, Trails
Committee
Members of the Trails Committee were present to discuss the extension of the Town-Wide
Public Trails Maintenance Permit, which was written by the Conservation Commission in
2009. A majority of the maintenance performed by the Trails Committee, and as part of
Scout projects, are removal of trees that have fallen across paths, pruning of branches that
protrude into paths, clean up litter, repair or replace boards on boardwalks and bridges,
clear drains, and DPW performs limited mowing. Mr. Tirane noted that communication with
the Trails Committee has been really good. Projects and maintenance are discussed at
Trails Committee meetings, documented in the meeting minutes, and recorded with the
Town Clerk. The Commission and Trails Committee briefly discussed how Pinevale
Conservation Area is the worst when it comes to Illegal leaf dumping. Ms. Longley
requested that the Conservation Commission be notified anytime there is a change in Chair
of the Trails Committee. The Permit will be updated with the following changes: change
wetland buffer zone on page 18; make sure wetlands are included with flood plains (G); and
change "Commission" to"Applicant" to notify property owners on page 18 (H).
Motion to approve extension of Town-Wide Trails Maintenance Permit by Mr.
Pinette, second by Mr. Flynn. Vote 5-0-0.

Mr. Tirone noted there were no updates since the last meeting.

No updates at this time. The grass has not yet grown in.
Administrator's Report
Mr. Tirane reviewed the history of oil tanks discovered at 287-285 Main Street. Before the
current project, there was an Order of Conditions that had to do with cleaning up some
underground oil tanks. The previous property owner completed the work and restoration of
the stream bank. About a month before he came to the Commission, more tanks were
discovered, so only a partial COC was issued. In order to move forward with financing, the
current property owner, Max, needs a completed COC. The site professional has issued a
close out on It and is supposed to submit a letter to Mr. Tirone. The Commission discussed
discrepancies in the reporting of the contamination to the DEP. The site professional at the
time only sent the reports to the Fire Chief, and not to the DEP. The OOC requires the site
be cleaned up. If the DEP has a close out, then the Commission can issue the COC.
However, the Commission agreed to wait to hear back from DEP.
