Commission reviewed status of work to stabiliz�ite after pool removal. A letter will be sent for the pending transfer of ownership. Pondview Lane Subdivision, Order of Conditions, DEP 270 -345, RGB 2002 -52 The Commission reviewed revised site plans for Lots 6, 9, and 10. The plan for Lot 9 needs further revision. Mr. Finch moved to accept the plan for Lot 6 as an insignificant plan change under the Order of Conditions. Ms. Meier seconded. Voted 5 -0 in favor. Mr. Hecht moved to accept the plan for Lot 10 as an insignificant plan change under the Order of Conditions. Ms. Longley seconded. Voted 5 -0 in favor. 4w, 40 Azalea Circle, Order of Conditions, RGB 1991 -3 The owner will be contacted to provide additional information necessary for an Extension. Public Hearings and Meetings 532 Pearl Street, RDA, RGB 2004 -17 The owner, Brian Walsh, presented a plan for driveway expansion. The Commission discussed details of proposed grading to prevent excess runoff onto abutting property. The Commission reviewed and amended draft conditions. Ms. Longley moved to accept the plan and issue a negative Determination with the conditions as amended this evening. Mr. Hecht seconded. Voted 5 -0 in favor. . Peter Gammie, project engineer, presented revised site plans, soils test results, and amended drainage designs. He also discussed the erosion barrier location, planting plan, and proposed walking path. The Commission asked for information about the proposed removal of the debris piles. They asked the Administrator to meet with the Town Engineer and project proponents to review the revisions. 4W Ms. Longley moved to continue the hearing to May 26, 2004 at 8:15 PM. Mr. Hecht seconded. Voted 5 -0 in favor. Minutes, Reading Conservation Commission, May 12, 2004 1 General Avenue, ANRAD, DEP 270 -421, RGB 2004 -6 The Chairman opened the hearing. The Commission reviewed and amended a draft Attachment for the ORAD. Mr. Finch moved to issue the ORAD with the Attachment as amended. Mr. Hecht seconded. Voted 5 -0 in favor. 468 West Street, 6 Kelch Street, and 11 Longwood Road, NOI, RGB 2004 -10 The Chairman opened the hearing. Bill Bergeron, Hayes Engineering, and Brad Latham, attorney, were present for the applicant. Mr. Bergeron presented the results of soils tests conducted earlier in the day and described revisions needed for the drainage design as a result of the tests. Proposed work can be eliminated from the RFA, and the water budget can be maintained to the B -series wetland. Additional information about landscaping was submitted. The Commission asked that native species be used. Ms. Fink reviewed a memo to the Commission and planned to meet with the Town Engineer to review the plan changes. Mr. Finch moved to continue the hearing to June 9, 2004 at 7:30 PM. Ms. Longley seconded. Voted 5 -0 in favor. Walkers Brook Crossing, Request to Amend Order of Conditions, DEP 270 -352, RGB 2001 -14 The Chairman opened the hearing. Mark Nogueira, VHB, presented a revised waste relocation plan. Ed Shaw, Dickinson Development, presented a revised construction sequence and reported the status of work completed to date. Phase 1 is nearing completion and Phase 2 is proposed to commence. DEP recently released $750,000 of the bond they have been holding for the capping. Ms. Fink compared the original plan to the new revisions, which has more work proposed in the ZNV. She raised questions about the construction sequence. Brad Latham, attorney, requested a waiver of the ZNV regulation and gave reasons that the work is necessary. Mr. Nogueira explained technical aspects of the design and constraints due to the capping of the landfill and public safety. He reviewed the planting plan for the ZNV. Mr. Finch moved to grant waivers for the reduction in the ZNV as shown on the revised plans. Mr. Hecht seconded. Voted 5 -0 in favor. Mr. Iacoviello, owner, presented a plan for a pool, deck, and patio. 2 Minutes, Reading Conservation Commission, May 12, 2004 The Commission reviewed site visit reports and questioned the BVW delineation. They asked for clarification about which trees the owner wants to cut. The Commission reviewed and amended draft Findings and Conditions. Ms. Longley moved to accept the plan and issue a negative Determination with the conditions as amended this evening. Mr. Hecht seconded. Voted 5 -0 in favor. Meadow Brook Golf Club, 292 Grove Street, NOI, RGB 2004 -18 The Chairman opened the hearing. Joe Orzel, Gulf of Maine, and Warren Cochrane, MBGC, presented a plan for renovation of the pool house, abandonment of a septic system, and connection to the public sewer. They do not know the exact location of the septic system at present. A sewer pump chamber will be required and will be installed in the basement of the pool house. The Commission asked whether the stream was perennial. Mr. Orzel said the watershed was less than 0.5 square mile. Ms. Fink noted that proposed work was more than 100 feet from the stream. The Commission questioned several wetland flag locations, noted an old dumping ground and some erosion problems. John and Debbie Powers, 256 Grove Street, supported the sewer tie -in and asked about the proposed land swap to expand the golf course. They were concerned about traffic increases. They asked for clarification of the location of the new pool house. Mr. Cochrane said the board of governors had recently voted not to proceed with the expansion and the membership will vote soon on the matter. He showed the proposed pool house location. The Commission noted that the site was in the Aquifer District. Mr. Cochrane said that a roof infiltration system could be included. Ms. Fink recommended soils tests. Mr. Finch moved to continue the hearing to June 9, 2004 at 7:45 PM. Ms. Meier seconded. Voted 5 -0 in favor. Old/New Business 28 Francis Drive, drainage complaint Ms. Fink presented a letter dated May 7, 2004, from the owner's attorney, denying access for an inspection of the site. Mr. Lachava, 38 Francis Drive, described the history of the swale and his observations of flooding since 1976 when he moved in. He stated that the flooding that occurred up- gradient of #28 during the storm on April 1 was more extensive than ever before and blamed the problem on a fence and a thick layer of stone fill in the swale at #28. He presented photographs and reported that he ran 3 pumps for about a week to lower the (W water. He reported that the flood had affected his shed and a retaining wall at his pool. Mr. Finch moved to issue a Certificate of Compliance for the work. Mr. Hecht seconded. Voted 5 -0 in favor. 20 Melendy Drive, OOC, DEP 270 -391, RGB 2003 -8 The Commission reviewed a foundation as -built plan. Ms. Fink will inspect the site. 422 Grove Street, OOC, DEP 270 -406, RGB 2003 -33 The Commission reviewed a revised site plan. Mr. Finch moved to accept the revision as an insignificant plan change under the OOC. Mr. Hecht seconded. Voted 5 -0 in favor. 306 Main Street, OOC, DEP 270 - 270 -356, RGB 2001 -19 Mr. Hecht moved to issue and Extension Permit for three additional years. Ms. Meier seconded. Voted 5 -0 in favor. (W 9 Mill Street, North Reading, OOC The Commission reviewed an OOC issued by the North Reading Conservation Commission for work in the RFA at 9 Mill Street. The Commission also reviewed a draft appeal of the decision. Mr. Finch moved to submit the appeal to DEP as drafted. Mr. Hecht seconded. Voted 5 -0 in favor. 40 Harvest Road, OOC, DEP 270 -417, RGB 2003 -53 The Commission reviewed the foundation as -built plan. Ms. Meier moved to accept the plan. Ms. Longley seconded. Voted 5 -0 in favor. 18 Symonds Way, 321 Haverhill, 315 Haverhill, Symonds Way Ballfield, 51 Cross Street The Commission received site visit reports from Ms. Fink. 30 Chequessett Road, MP Mr. Hecht moved to accept the Minor Project permit. Ms. Meier seconded. Voted 5 -0 in favor. 187 Sanborn Lane, MP Mr. Hecht moved to accept the Minor Project permit. Mr. Finch seconded. Voted 5 -0 in kw favor. 4 Minutes, Reading Conservation Commission, May 12, 2004 294 Main Street, Enforcement Order Ms. Fink gave a site visit report. The Commission directed her to contact the owner and engineer to resume the site work at this time. 118 Fairchild Drive, DA Mr. Finch moved to authorize the Admininstrator to sign off on the Certificate of Occupancy, pending completion of satisfactory stabilization under the deck. Ms. Meier seconded. Voted 5 -0 in favor. Lob's Pound Mill, OOC, DEP 270 -418, RGB 2004 -2 Ms. Fink reported that DEP has issued the Chapter 91 approval. MBTA Ms. Fink reported receipt of a Yearly Operational Plan for herbicide use. She will check the status of the Determination. 37 Latham Lane, OOC, DEP 270 -357, RGB 2001 -20 Ms. Fink reported receipt of a new status report for the on -going hazardous material clean-up. Work is proceeding in order. Friends and Family Day The Commission made plans for a booth at this event, scheduled for May 22. The meeting adjourned at 11:00 PM.
