May 24, 2012 .1111 SEP 24 P f= The meeting of the Reading Board of Health came to order at 5:30 pm in the Conference Room of Reading Town Hall. Present were Board members Barbara Meade, David Singer, Andrew Friedmann, staff Ruth Clay and David Greenbaum, public Matthew Caraco, and Macaroni Grill store manager John O'Brien. Minutes Minutes were tabled as there was a section that was not completed. Well Application — 3 Dividence Road Matthew Caraco, the owner of 3 Dividence Road, was present. The Board had no questions of the applicant. Motion made by Meade, seconded by Friedmann to approve the application. Vote passed 3:0:0. Macaroni Grill Inspection Review Andrew Friedmann recused himself from this agenda item. Clay gave a summary of inspections and meetings with the management of Macaroni Grill, including a meeting with the store and district manager January 4, 2012. The Board reviewed copies of the last three routine and follow up inspections. O'Brien gave a summary of efforts to correct violations and maintain the operations. Greenbaum gave a summary of his actions. The Board discussed the situation and voted 3:0:0 to order Macaroni Grill to hire an independent consultant within ten days to assist them in improving their operations. A copy of the Scope of Work and Timeline is to be sent to the Health Director along with copies of progress reports. Public Hearing — Well Regulations Motion was made by Singer, seconded by Friedmann to open the Public Hearing. Unanimously approved. There were no comments from the public. Motion was made by Singer, seconded by Friedmann to close the Public Hearing. Unanimously approved. Andrew Friedmann asked if the 2 1 E sites were mapped so that proposed well locations could be checked against these sites. Clay said she would look into that for the next meeting. Clay pointed out additions from the MassDEP Model Regulations requested by the Town Engineer. Motion by Friedmann, seconded by Singer to approve the revised Well Regulations. Motion passed unanimously. Public Hearings Sections 16 and Section 25 Tabled to next meeting as letters had not been sent to existing establishments which sell tobacco products. Other business and items not reasonably anticipated at the time of the posting • Clay noted that due to impending construction at the offices of Community Services this weekend, she moved the vaccine to her home refrigerator to ensure no loss of power. • Clay gave Board members information about a breakfast Mass in Motion meeting scheduled for policy makers June 4, 2012 at the Wakefield Americal Civic Center. Meade asked for that information to be sent to Board members via e- mail.
