' The Reading Retirement Board held a meeting on May 26, 2004, in the Accounting and Finance Department Office, To8 8 3 <br />Hall, Reading, Massachusetts. Salary <br />The board unanimously approved the request of Salvi LaVita, a police officer, to redeposit funds that were refunded to him <br />by the State Board of Retirement on 3/2/01. Upon proper repayment to the Reading Retirement System, the State Board of <br />Retirement will accept liability for 1 year, 11 months and 13 days of creditable service. The cost to redeposit is $6,056.14. <br />Salvi will have deductions taken from his bi- weekly paycheck, in the amount of $47. This will result in his balance being <br />paid off in about five years. <br />The board voted unanimously to send a letter to Robert Toussaint's attorney, Paul Danahy, informing him that "The board <br />voted at the May 26, 2004 meeting to hold an investigatory hearing pursuant to 840 CMR 10:09 on Mr. Toussaint's <br />accidental disability retirement application. The board has set aside two dates, July 7 and July 8, 2004, for the hearing, <br />whichever is agreeable to you and your client." <br />Donald Feltch, who worked for RMLD and retired on February 28, 1978, passed away on 5/18/04. He chose Option B at <br />his retirement but there are no annuity funds remaining. He retired on an accidental disability retirement. His spouse, <br />Anne, is eligible for a Section 101 allowance of $6,000 per year because Donald retired prior to November 7, 1996. <br />Irene Feamside chose Option C at her retirement and her spouse has passed away. PERAC has included in their records the <br />payment of an Option A retirement allowance to Ms. Feamside (Option C "pop -up "), effective as of March 24, 2004. <br />Andrea Fitzpatrick, who has applied for an accidental disability retirement, has been scheduled for a joint medical panel <br />exam on June 10, at 3:00 PM. <br />Wayne Harding, who retired on a superannuation retirement last May 2003, has filed an application for an accidental <br />disability retirement. Since Mr. Harding has been employed by the Town of Reading RMLD for only a few years, the <br />board instructed the board administrator to ask the City of Peabody for copies of any worker's compensation records and <br />injury reports they have for Mr. Harding. In addition to these records, the board administrator should request from RMLD <br />the medical records of Mr. Harding's pre - employment physical and get information from Mr. Harding about the surgeries <br />he had back in 1972 and 1974. <br />On May 4, 2004 PERAC notified the board that Thomas Morgan was evaluated for possible return to service and PERAC's <br />internal Nurse Case Manager determined that he is unable to perform the essential duties of his job. The board is not <br />required to take any action at this time. <br />
